The form entitled “Prescribed Form of Summons Under Subsection 33 (4) of the Act”, labelled as Form 1 and dated 2011/04, that is available on the website of the Government of Ontario Central Forms Repository under the listing for the Ministry of the Attorney General is prescribed as the required form for a summons issued under subsection 33 (3) of the Act.
