For information purposes,
the names and locations of the clinics in Ontario are set out in Table 1;
the access zone established under section 6 of the Act for each clinic is the property on which the clinic is located and the area around the property, as described in Table 1, subject to subsection 6 (5) of the Act; and
for each property referred to in clause (b) on which a clinic is located, Table 1 sets out the Property Identification Number for the property and the Land Registry Office number for the Land Titles Division or Registry Division in which the property is located.
