A summons issued under subsection 12 (1) of the Act shall be in the form entitled “Summons”, labelled as Form 1 and dated November 2014, that is available on the website of the Government of Ontario Central Forms Repository. ;

A warrant issued under subsection 12 (4) of the Act shall be in the form entitled “Warrant for Arrest (Defaulting Witness)”, labelled as Form 2 and dated November 2014, that is available on the website of the Government of Ontario Central Forms Repository. ;
